You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11.
You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.
You may be eligible to use this form if you need to change or correct your passport, or you need to replace a passport that was limited to less than 10 years validity (which is the normal validity period for an adult’s passport).
Your photo is a vital part of your U.S. Passport application, and providing an unsuitable photo can result in delays in processing your passport. You must provide one photo on a white background with your passport application. To enhance the security of the application process and verify your identify more quickly, you must remove your eyeglasses for U.S. passport and Visa photos. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants. The acceptance of your photo is always at the discretion of the U.S. Embassy. We recommend you use a professional passport photo service to ensure your photo meets all the requirements.
Use the Department of State’s passport application system to find the form you need. Other sources might have outdated versions of the forms that will not be accepted. If you print your forms, make sure the form is sized to fit your paper. If part of the form is not printed, the form will not be accepted. You should fill the form using blue or black ink. Forms filled with other colors may not be accepted. Fill your forms carefully and completely before your appointment. Incomplete forms and forms with incorrect information will not be accepted. In some cases it may not be possible for you to complete or correct forms during your appointment, and you may be required to reschedule on another day. Generally, immediate family members may accompany passport or CRBA applicants to their appointment interviews at a U.S. embassy or consulate, and all minor children must be accompanied by a parent or guardian. Passport or CRBA applicants also have the option of being accompanied by an attorney at their appointment interview. Attendance by any third party, including an attorney, accompanying an applicant is subject to the certain parameters designed to ensure an orderly appointment interview process and to maintain the integrity of the adjudication of the application.
Minors under the age of 18 must use form DS-11 to apply for passports. They are not able to renew passports using form DS-82.
